Trip Information & Liability Waiver & Release Form (doc)- This form is used when you want to sign up for a trip and gives us information about yourself, so we can book your trip
Scuba Activities Liability Waiver & Release Form (doc) - This form is used when you want to sign up for any Scuba Houston scuba organized diving activity in the shop or out on a location

Emergency Treatment Consent Form (pdf) - This form is for parents to
give Scuba Houston authorization to obtain or provide medical assistance to
a minor when parent or guardian is not present

Planning- This form should be used as a checklist for all dives.
Recreational Scuba Diver's Physical Examination Guidelines (pdf) - Use this form to help your Physician with the examination requirements
Medical
Examination Form (pdf) - This form gives you a checklist to evaluate
your physical situation and has a place for your Physician to fill in his report

Trip Survey (doc)-
Upon completing a trip, Scuba Houston Divers should print out this form and
return it to Scuba Houston.
Activity
and Model Release Form (doc)- Divers on all trips should complete
this form when pictures are taken, so we may put them on the web.
